Form 1095-C/1095C for the 2025 Tax Year
In accordance with federal regulations, Roswell Independent School District is not required to automatically mail Form 1095-C to all employees. Form 1095-C provides information about the health insurance coverage offered to you during the year.
How to Request Your Form:
Full-time employees and covered individuals may receive a copy of their Form 1095-C upon request. To request your form, please contact us using one of the following methods:

Timing:
We will provide your requested Form 1095-C within 30 days of receiving your request.
Note: You do not need to wait for Form 1095-C to file your individual income tax return.
